Health Screening

The following questions have been designed to identify the small number of people for whom it would be wise to have medical advice before starting.
Has your doctor ever said you have a heart condition?
Do you feel pain in your chest when you do physical activity?
Do you ever lose balance because of dizziness or ever lose consciousness?
In the past month, have you had pain in your chest when you were NOT doing physical activity?
Do you have a bone or joint problem that could be made worse by a change in your physical activity?
If you answer yes to any of the above questions, we recommend you seek medical advice before starting your cycle training. By signing the cycle training consent form you acknowledge that you undertake cycle training at your own risk.

Please read and agree to the Ridewise consent form:

I agree to the following points of information and I will take lessons in safe, effective cycling.

Except in cases where Ridewise supplies the bicycle I ride I understand that
  1. The instructor(s) may refuse to train me if they decide my bike to be unroadworthy. Their decision is final. I understand that not being able to commence and/or continue with a training session due to having an unroadworthy bike will not entitle me to a refund of any fees paid.
  2. It remains my responsibility not to ride an unroadworthy bike. If I am in doubt as to whether a bike is road worthy I should seek the advice of a professional mechanic.
  3. Ridewise trainers will not carry out repairs to make an unroadworthy bike roadworthy.
  4. It is my responsibility to ensure that my bicycle is roadworthy before attending the training session.
I accept that the instructors may at any time refuse to continue to train me if my behaviour or ability level is deemed to be unsuitable.

I understand that RideWise is not responsible for any injury or loss or damage to any property which is not caused by an instructor’s negligence and that RideWise takes no responsibility whatsoever for any injury or any loss or damage to any property which occurs either on the way to or from a lesson or subsequent to lessons.

I understand that having taken training it does not necessarily follow that it is safe for me to ride a bicycle. To become proficient takes much more practice than lessons of this kind can provide.

RideWise leaves the decision about wearing a cycle helmet entirely up to me. The decision to wear a helmet is a purely personal one. RideWise will not be held liable for the results of any injury, which is later established, may have been prevented by the wearing of a helmet. If a helmet is worn Ridewise will not be held liable for the results of any injury which it is later established may have been caused by the wearing of my helmet. The instructor will advise me only on the fit of the helmet but will not necessarily be able to judge from its appearance if it is in good condition.

I undertake to make the instructors aware of any medical condition I have which may adversely affect the training.

I understand that personal details held by Ridewise may be given to the organisation that is in whole or in part paying for my lesson and that the organisation may contact me to ask for details of the lesson.

I understand that occasionally it may be necessary to take images of those undertaking cycle training and that they may be used for publicity purposes.

Instructors are not at liberty to amend the terms of this consent form.

I understand this consent form must be completed and signed before a training session commences and that a training session cannot take place if this consent form is not signed.

This consent form applies to any training organised by Ridewise. I understand a new consent form need only be signed for any sessions that occur after the terms vary.

I understand that if I have answered ‘Yes’ to one or more health screening questions, I should seek medical advice before attending a cycle session. I agree to tell the instructor if there is a change in my medical condition. I understand that this information will be shared with other cycle instructors and that I cycle at my own risk.

Please read the cycle loan conditions if you wish to loan a cycle:

1. The instructor(s) may refuse to train me if they decide my bike is not roadworthy. Their decision is final. I understand that I will not be entitled to a refund of any fees paid if I am unable to start or continue a training session because my bike is not roadworthy.
2. The cycle shall not be disposed of or leant to another person.
3. The cycle shall be locked securely at all times it is not in use. The cycle should be locked to a secure item. When at a person’s home the cycle should be kept in a locked garage or locked shed if such a facility exists and should also be locked.
4. If the cycle should be stolen then the person should immediately report the theft to the police and, obtain a crime reference number. Ridewise must immediately be notified in writing of the theft and crime reference number and provide Ridewise with any other information that may reasonably be required.
5. If the cycle should be damaged Ridewise shall immediately be informed in writing of the nature of the damage and the circumstances that caused the damage. If the damage affects the safety of the cycle it shall promptly be returned to Ridewise so that any necessary repairs can be effected. The person should not ride the cycle where damage affects the safe use of the cycle. Ridewise shall be responsible for ensuring that the cycles are properly repaired.
6. Ridewise does not have insurance to cover the use loss or damage of the cycle whilst in the possession of another person.
7. The maximum period of the loan of the cycle shall be 1 month from the date it is collected from storage. Prior written agreement with Ridewise can extend this. It is the responsibility of the person borrowing the cycle to collect it from the place of storage. It is the responsibility of the person borrowing the cycle to return it to storage either at the end of the one month period or at an agreed later stage or promptly whenever requested by Ridewise. Ridewise reserves the right to request orally or in writing the return of the cycle earlier than 1 month.
8. The person borrowing the cycle is the person undergoing adult cycle training.
9. All keys and locks and any other accessories shall be returned with the cycle at the end of the loan period.
10. The person borrowing the cycle agrees to abide by any of the arrangements of the storage company when on their premises.
11. These terms are cannot be amended without specific prior written approval from Ridewise.
12. The bikes are stored at Groundwork Greater Nottingham, Denman Street East, Nottingham NG7 3GX
(We are situated between Ilkeston Road and Alfreton Road)
13. Phone number is 07854 853339
Please ask for Graham Hubbard.
14. The opening hours are as follows:
Monday to Friday 8.30-4.45pm;
Unfortunately we are not able to arrange for collection/dropping off the bikes at the weekend.
15. The person collecting the cycle shall arrange with Graham Hubbard the date and time of collection. Someone from Groundwork Greater Nottingham or Ridewise shall then meet the person borrowing the cycle and arrange for the cycle to be collected. The person borrowing the cycle shall be asked to provide proof of identity if so requested, and to sign any receipt for collection of and return of the cycle as may be requested. Before collecting and/or returning the cycle the person borrowing it shall check it to see if there is any obvious damage and to note that on the cycle defect form.
16. Please select a cycle from the ones that are available. Ridewise cannot guarantee to have a specific cycle available. Ridewise only has a few cycles and they are of different sizes. We hope to be able to provide a cycle for someone when it is needed but this cannot be guaranteed. It is probably a good idea to ring Ridewise or groundwork Greater Nottingham to ascertain what cycles are available and their sizes before going to try and collect one. If the cycles available do not fit you it could be in your best interests to wait until one that does fit is returned.
17. For the avoidance of any doubt written permission includes email.
